Radio Equipment Checkout & Use Acknowledgment
(Employees, Independent Contractors, Vendors, and Partners)

This Radio Equipment Checkout & Use Acknowledgment (“Acknowledgment”) is entered into by and between Headline Inc. (“Company”) and the undersigned individual (“Signer”). This Acknowledgment confirms receipt, responsibility, and proper use of Company-issued radio equipment, and includes consent to receive operational communications.

1. Issued EquipmentI acknowledge receipt of the following equipment, which remains the sole and exclusive property of Headline Inc.:

Motorola XPR3500e Digital Two-Way RadioMotorola
PMMN4013A Remote Speaker Microphone
Motorola PMLN5727A Surveillance Earpiece (if issued)

Equipment is issued temporarily for authorized work-related use only.

2. Care, Use, and SecurityI agree to:

Use the equipment only for authorized work purposes

Exercise reasonable care at all timesKeep the equipment secure and attendedNot loan, modify, or transfer equipment without authorizationImmediately report loss, theft, or damage to Company managementResponsibility begins at checkout and ends only when the equipment is returned to and accepted by Company management.

3. Loss, Theft, or DamageI acknowledge that loss, theft, or damage beyond normal wear caused by negligence, misuse, or failure to follow Company procedures may result in consequences depending on my classification, as outlined below.

4. Independent Contractors, Vendors, and Partners

Financial ResponsibilityIf I am engaged by Headline Inc. as an independent contractor, vendor, or partner, I acknowledge and agree that:I may be held financially responsible for equipment that is lost, stolen, or damaged due to negligence or misuseI agree to reimburse Headline Inc. for the reasonable replacement cost of any such equipment, including while working outside the State of Colorado

Reimbursement will be handled outside of payroll through invoicing or other lawful meansReplacement Costs:

Motorola XPR3500e Radio: $794.99
Motorola PMMN4013A Remote Speaker Microphone: $55.49
Motorola PMLN5727 Surveillance Earpiece: $34.65

Replacement cost is defined as the reasonable cost to replace the equipment with comparable, working equipment and may vary based on market availability.

5. Employees – Acknowledgment of ResponsibilityIf I am engaged by Headline Inc. as an employee, I acknowledge and agree that:I am responsible for exercising proper care and professionalism when using Company equipmentI understand that Colorado law generally restricts payroll deductions for equipment loss or damageThis Acknowledgment does not authorize automatic wage deductionsFailure to properly care for, return, or report issues with equipment may result in disciplinary action, up to and including termination, in accordance with Company policy and applicable law

6. Communications Consent (Text & Phone)

I consent to receive calls and text messages from Headline Inc. related to:Equipment checkout, return, loss, or damageOperational communications related to events, shifts, or assignmentsTime-sensitive or safety-related informationMessage frequency may vary. Standard message and data rates may apply depending on my wireless carrier. This consent does not authorize marketing or promotional messaging and is not a condition of employment where prohibited by law.

7. No Transfer of OwnershipNothing in this Acknowledgment transfers ownership of any equipment. All equipment remains the exclusive property of Headline Inc. at all times.8. Return of EquipmentI agree to return all issued equipment:At the end of my shift, assignment, or eventImmediately upon request by Company managementIn the condition it was issued, normal wear exceptedFailure to return equipment may result in further action consistent with my role and applicable law.

9. Governing LawThis Acknowledgment shall be governed by and interpreted in accordance with the laws of the State of Colorado, regardless of where work is performed.

10. Acknowledgment and Agreement

By signing this form upon checking out your radio and accepting the equipment, I acknowledge that I have read, understand, and agree that this Acknowledgment constitutes a binding agreement between myself and Headline Inc.
